1. The employer has not effectively developed or implemented an Infectious Disease Preparedness and Response Plan, including but not limited to: a. Social distancing is not maintained at the location. b. Masks or face covering are not used systematically by employees and management. c. Employees showing symptoms of COVID-19 are not quarantined or are discourage to quarantine. d. Contact tracing is not done with employees potentially exposed to COVID-19 positive cases. 2. The employer has not provided adequate sanitization at the facility, including common areas as well as work areas, after confirmed COVID-19 positive cases.
