1. High contact areas on the Platform are not being cleaned and disinfected in accordance with CDC guidelines. The entire Platform Area is not being cleaned and disinfected on a daily basis. The Bay Doors on the platform are not being cleaned and disinfected on all three tours. The Bay doors, rails, and doors where drivers go in and out not being cleaned and disinfected at any level. 2. COVID-19 safety talks are not being documented with employee attendance documentation (sign offs). Employees absent from the COVID-19 safety talks are not accounted for and subject to make-up training. 3. Employees are required to work closely with worker that have tested COVID-19 positive and the COVID-19 positive employees are not required to self-quarantine. 4. A Tour 2 Expeditor tested COVID-19 positive and two other Expeditors and a Supervisor that were in close contact with the positive subject were not notified. The facility nurse and plant manager were notified about the Tour 2 Expeditor, however, no other notification of this positive case was communicated to other workers. 5. Employees that are reporting ill are not directed to stay home and quarantine in accordance with CDC guidelines. 6. Personal Protective Equipment (PPE) supplies are not available in all work areas including the main Platform area. 7. STC Areas, in addition to the Platforms, are not being routinely cleaned and disinfected. 8. Several employees in STC Area tested positive for COVID-19 on February 1, 2021 and affected employees were not notified. On February 3, 2021, the Tour 2 Expeditor on Platform STC Area tested COVID-19 positive, however, only two other Expeditors were never notified on Tour 2 and directed to quarantine and no other employees were informed of the positive case and potential exposure. Additionally, a mail handler on Tour 2 in the Platform STC Area tested positive for COVID-19 and no one had notified the other employees who had close contact with this handler.
