a) When the breaks on carts don't function properly the cart is put back into service without repairing or replacing the breaks. The breaks are removed as a quick fix and this leaves the carts not fully functional. The carts then become unsafe to use since they can't be secured in a stationary position. The carts may roll uncontrollably and this poses a risk.
