1- There have been numerous employees as well as residents who have become ill from COVID-19. 2- Employees who have worked with suspected and confirmed cases of COVID-19 residents have not been provided n95 or higher levels of respiratory protection. 3- A respirator program including medical clearance, fit testing, and training has not been developed and implemented. 4- Staff is very limited. There have been shifts without any CNAs on the units. 5- Due to low staffing employees have been at risk for injuries including musculoskeletal injuries. 6- The same staff are working COVID-19 and non COVID-19 units.Alleged Hazards: 6, Employees Exposed: 50
