1.On 12/18/2020 the employers PII and PII held a large meeting in the warehouse where masks were not required. 2.After the meeting one of the employees (PII) left stating he had Covid. 3.On January 4th another employee (PII) who had claimed had tested positive for Covid, was asked to come into work at the site location. 4.He was in the office (3 people were in the office) and in the ware house (1 other worker was in the warehouse) where he was around others and not wearing a mask. 5.The operations manager called PII on Jan 4th around 9AM asking if he knew why PII was there. 6. PII was visibly ill. He was pale and coughing which was noted to the owner. 7. PII claimed he didn't know if PII was sick or not. 8.He did not ask PII to go home or put a mask on. 9.There is no thermometer to take an employee's temp. The owners required employee's to physically attend meetings, but do not require them to wear masks. Surfaces are not cleaned after exposure. Only a few surfaces are wiped down one time during opening in the AM and once during closing at the end of the day. If the person who is assigned to cleaning calls out sick, the job is not completed to wipe and sanitize.Source: Osha.gov | Receipt Date: 2021-01-07
