1. JSA are not being completed properly to identify potential hazards for the particular job or task. (The employer is doing one general JSA and employees are tasked to do several jobs/tasks.) 2. During the morning safety briefs employees are asked to gather without properly social distancing, exposing employees to COVID-19. 3. Face shields are not being sanitized prior to use. (Employees share face shields.) 4. Boom Lifts are being modified with 2x4's to lift paneling (paneling measures 17ft or 27ft in length and weighs approx. 150 lbs.) (Two employees will get on the boom lift, one will control the boom and the other will hold the paneling.) Employer not following manufacturer's capacity limits and panels being lifted up have fallen off the boom a least twice while using this operation. 5. Supervisors do not practice safety aerial lift procedures even though it is briefed in the mornings not to go underneath loads, supervisors do it continuously and in the presence of safety personnel.
