1) Staff screening temperature is being done by staff without masks on. 2) The staff conducting the screening are not asking about symptoms or checking for the checkmark for 7 weeks now. 3) The employer is refusing to allow any off site work for many departments including completing paperwork. Location: suite G12, ART department. 4) The employer is refusing to allow "pods" in specific departments to reduce staff members which exceeds the allowable number of persons in a space at one time on a daily basis. Location: ART department. 5) The employer does not supply hand sanitizer to the departments, staff, clients, or at entrances for employees. 6) The employer does not inform staff of the clients they will be in contact with (in person) when they are Covid-positive or having symptoms. Location: ART department. 7) At the main entrance, the employer does not read signage to those who cannot read or provide translating for those who do not speak or read English. 8) The employer is allowing visitors into the building who are not necessary support people or guardians without clinical reasoning. 9) The employer does not have an on-site cleaning crew. 10) There is no cleaning of touch points during the day, bathrooms, areas of use, waiting rooms, or have staff available after 12pm to call when needed. This is more so on the ground level where all clients are seen in person during full business hours. 11) The employer does not supply office space allowing for 6 feet of distance when meeting with clients, will not supply plexiglass barriers, wipes for pens or clipboards, or related. Larger areas are available but are not allowed by management. Location: ART department. 12) The employer does not supply offices to meet with clients that meet basic safety and security needs.
