1. Bathrooms in the facility are unsanitary and adequate handwashing facilities are not available. 2. The warehouse is not heated and not kept at or above the minimum required temperature. 3. The employer is not requiring or enforcing the wearing of face coverings for employees as required in the COVID-19 Preparedness Plan for MN Business guidance. 4. The employer does not have procedures in place for informing and allowing for employees to isolate or quarantine if there is a positive case or close contact with a positive case of COVID-19. 5. The employer is not providing personal protective equipment (PPE) needed by employees to conduct their work safely.
