1. Employees are not provided with N95 respirators when spending extended periods with COVID-19 patients or PUIs in small exam rooms, and when obtaining COVID swabs. 2. Fit testing being conducted for N95 filtering facepiece respirators is not in accordance with Appendix A of 1910.134, including fit testing multiple employees using the same respirator, and fit testing employees with facial hair. 3. Employer has stated that employees can use PAPRs instead of N95 respirators, but only four PAPRs were provided to the clinic, and only two are operational. Employees have not received training on how to inspect, put on and take off the respirators, or how to disinfect the PAPRs. 4. Employees are not screened for fever or symptoms before or when they report to work. 5. Employer not conducting contact tracing of workplace exposures to COVID-19. 6. Lobby registrars are not distanced from patients and sneeze guards have not been installed. 7. Employer does not have procedures in place to ensure patients and parents of patients wear face coverings while in the clinic.
