316 Indian Ridge Boulevard, Mishawaka, 46545 Indiana, United States

Employer not disinfecting/deep cleaning work place. Other: I am a vendor that delivers to Walmart on a daily basis and hanging down in their receiving door way, that every vendor has to walk in and out of, are these large plastic strips that are not being taken down or even cleaned. It is a hazard for every vendor, who is trying to serve our community, that passes through those unsanitary plastic strip. I don't know where else to turn for the protection of myself, my family, my employees and every other person that passes through that doorway. ER, Mishawaka Mayor's Office, and Health Department have been notified of alleged hazards.

I did not order a package that I received from Heather Mottysy. There was 2 identical items in it that I think was some kind of arm or elbow brace but they were thin and cheaply made. The material even had a strong odor to them.
