1.) Employees are not provided with PPE, masks and/or face covers for the prevention of / protection from COVID-19 transmissions. 2.) Areas/items including, but not limited to, cases, slots, time cards, scanners, and vehicles, have not been cleaned following potential contamination of the COVID-19 virus from employee who have test positive for the virus. 3.) Items potentially contaminated with the COVID-19 virus were brought into the building from a vehicle to contaminate the work area, items within the work area, and expose employees to COVID-19. 4.) Areas were not cordoned off to prevent exposure to the items brought into the building from a vehicle that could be potentially contaminated with the COVID-19 virus. 5.) Areas where items were brought into the building from a vehicle that could be potentially contaminated with the COVID-19 virus and the items themselves were not cleaned/sanitized to prevent employee exposure to the COVID-19 virus.Alleged Hazards: 5, Employees Exposed: 50
