1. The employer does not ensure that the downstairs toilet facilities are kept clean. Sewage backup has not been cleaned in two weeks. T8 CCR 3364(b) 2. The employer does not inspect the HVAC system for the breakroom bathroom at least annually and correct problems found during these inspections during a reasonable time. T8 CCR 5141(b)(1) 3. The employer fails to enforce social distancing after the manager leaves, during which time, from 10:00 pm until 5:00 am, the night manager regularly violates social distancing around night shift employees. T8 CCR 3205(c)(6) 4. The employer fails to enforce mask wearing after the manager leaves, during which time, from 10:00 pm until 5:00 am, the night manager refuses to wear his mask around night shift employees. T8 CCR 3205(c)(7) 5. The employer fails to maintain roll up doors in safe operating condition. Of 48 doors, about 36 of them have broken chains and are regularly opened and closed by forklift or by employees standing on pallets grabbing the door with a hook. Vertically operated doors are not securely connected to their lifting tackle. T8 CCR 3323(a) & 3328(g) 6. The employer fails to ensure that employees select an appropriate ladder and do not climb on pallets, which are not designed as ladders, to lower the roll up doors. T8 CCR 3276(d)
