1. For many months, due to halted construction, bare walls with electrical wires are in employee interfacing areas. This would include the front lobby, waiting area, conference room, first floor back hallway where employees clock in, back hallway where COVID drive thru testing station is located and negative pressure exam room on the first floor. Second floor hallways, supply closet and vaccine room also have areas of exposed wires for at least 6 months due to ceiling disrepair/missing panels. 2. Staff bathrooms are nonfunctioning on the medical floor for over 6 months with backed up toilets. 3. The employer has failed to train employees and maintain records for blood borne pathogens (BBP) and hazardous materials. New and old employees are not receiving required BBP and hazardous materials training. 4. The employer has failed to fully implement procedures for COVID-19 that prevent crowding in patient waiting areas and prevention of symptomatic or asymptomatic patients from accessing the second (medical) or third ( dental) floor without proper screening. There have been COVID positive and suspicious COVID patients exposing staff. There is poor ventilation and conditions in the dental suite on the 3rd floor. 5. The employer has failed to regularly clean medical and dental areas speciifcally, building overall per medical practice standards.
