1. The employer allows some employees and managers to work without a face covering or facemask required at the facility to prevent the spread of COVID-19. In addition, some employees are allowed to work wearing the face covering or facemask incorrectly, for example mask lowered over the chin. The employer is not following the CDC guidelines in regards cleaning and sanitizing the facility after employees have tested positive for COVID-19.
