1. The employer is exposing employees to respiratory hazards from lubricant oil mists from parts coming out of the machine without evaluating if respiratory protection is needed. 2. The employer is exposing employees to amputation hazards from shearing parts of the machines operated by employees in the three operational lines that do not have guards on them. 3. The employer is exposing employees to struck-by hazards by forklift operated by operators who have not been trained and certified. 4. The employer is exposing employees to chemical hazards from lubricant oil and other chemicals used by employees without providing employees with safety data sheets for the chemicals and hazard communications training. 5. The employer failed to ensure employees were practicing social distancing, maintaining six (6) feet distance, between persons within the facility or wear cloth face coverings when other social distancing measures are difficult to maintain as in accordance to CDC guidelines for the COVID-19 pandemic. 6. Employees are exposed to health hazards associated with COVID-19 in that the employer does not screen employees for symptoms of the virus per CDC (Center for Disease Control and Prevention) guidelines. 7. Employees are exposed to health hazards associated with COVID-19 in that the employer does not provide hand sanitizers and soap for employees to wash their hands as per CDC (Center for Disease Control and Prevention) guidelines.Alleged Hazards: 7, Employees Exposed: 12
