1. Recently, an employee tested positive for Covid-19. The employer did not notify all employees, that had close contact (within 6 feet for a total of 15 minutes or more in a 24 hour period) with the affected employee, that they may have been exposed to Covid-19. 2. An employee tested positive for COVID-19, and the facility is not being sanitized to protect the remaining employees. 3. Commonly contacted surfaces and shared tools are not routinely sanitized to prevent the potential transmission of Covid-19. 4. Employees do not wear masks when they cannot maintain social distancing while accomplishing their work.Alleged Hazards: 4, Employees Exposed: 100
