Regulatory Report

1545 East Flamingo Road, Las Vegas, 89119 Nevada, United States

1. Employees are concerned that current operations make it impossible to socially distance themselves from others per the Governor's order to prevent the spread of COVID-19. Each shift has a large volume of workers on duty.
2. Employees working in the front of the house are concerned with exposure to COVID-19 due to having to get too close to customers who are not wearing masks.
3. Employees in the store that are wearing masks are wearing them improperly to include not covering their nose or pulling the mask down to talk.
4. There is no hand sanitizer available for use for all employees.
5. Employees are concerned about the reuse of their surgical masks.

Hazard Location:
1. In the kitchen area and the front house area.

To my knowledge I don't have any outstanding orders from anywhere. Pkg came today in bag from USPS as it has been damaged and there are no contents. Says from Lucia at
4375 South Lee St in Buford, Ga.
I cannot find a single clue as to who this is from !
