1. The facility is continuing to have outbreaks of COVID-19 and the employer is not notifying the employees of any positive cases. The employees are finding out from each other and have concerns about being potentially exposed to the virus.

2. There is no social distancing. Employees working on lines are less than a foot from each other.

3. Employees are using the same time clock and restrooms and proper cleaning is not being done.
