complains that her bus was dead, contacted Maitenance/Mechanic they came to her bus without a mask in order to jump the bus. Violating policy of Mask Mandate while on property. Employees positive for COVID, no notice to employees and or quarantine of exposed employees. Mechanics rarely wear masks and never sanatize a bus after contact. Note: An additional anonymous complaint was received on 4/7/2021 and was included in this inspection while it was opened. The complaint included the following allegations: Everyday there are about 140 school buses riding at full capacity, which it makes it impossible to keep the required social distancing. The employer is telling the bus drivers that if a kid shows up without a mask to let the kid ride in the bus without it and not turn the kid away. The caller is concerned that bus drivers are exposed to COVID19 for not enforcing COVID regulations in the school buses.

room with her that they didn't need to quarantine. If they were wearing the personal protective equipment (PPE) they are fine. No office disinfection or sanitization was done. No patients or staff were notified. 2- We had a staff meeting this week. There was no social distancing. There were over 20 employees present. 3- The waiting room and the front counter are supposed to be wiped down and sanitized every hour. No one does it ever. It's not a safe office, way too many patients are seen at the same time. 4- We have a small lunch room. They don't limit the number of employees that are in there. One day there were 6 employees without masks eating lunch. There are no rules at this office. 5- There is no plexi glass at the front desk. We were told the company couldn't fund it. The employees that work at the front desk are within a couple feet of these patients.

Mechanics working on the buses without wearing masks, or following social distancing guidelines. The training staff are on the buses without wearing masks, or following social distancing guidelines. There are approximately 90 employees including bus drivers, office staff, trainers/trainees and mechanics at the facility, who are being exposed to Covid-19 by each other and by students. Employees are being exposed daily in multiple areas because of the failure to use face coverings and/or follow social distancing guidelines. Currently 6 employees have been exposed, with 4 quarantined. Covid-19 testing is not available to employees, and employees are not paid during quarantine after being exposed on the job. Cascade Student Transportation has continued to ignore COVID-19 and the guidelines to keep it's employees safe. CASCADE STUDENT TRANSPORTATION 1304 Se Eaton Blvd Battle Ground, WA 98604-3311

Aruba Aloe Hand Sanitizer Gel is used as a sanitizer to help reduce bacteria that potentially can cause disease and is packaged in 12 fl oz (355 mL) dark green plastic bottles with white label reading in part “ARUBA ALOE Hand Sanitizer GEL 80% Alcohol Made in Aruba World’s Finest Aloe”, with barcode 0 82252 03300 5.

Aruba Aloe Alcoholada Gel is used for temporary relief of pain and itching associated with minor burns, sunburn, insect bites, or minor skin irritations and is packaged in two sizes: 2.2 fl oz (65 mL) plastic bottles with barcode 0 82252 34030 1 and 8.5 fl oz (251 mL) plastic bottles with barcode 0 82252 03120 9. The plastic bottles are transparent with a label reading in part “Alcoholada Gel Pain Relieving Gel 0.5% Lidocaine Hydrochloride”.

Aruba Aloe Balm N.V. has notified all customers that bought these products by email and has offered a discount coupon for a next purchase. Consumers that have products which are being recalled should stop using and discard the product.

Risk Statement: Substantial methanol exposure can result in nausea, vomiting, headache blurred vision, coma, seizures, permanent blindness, permanent damage to the central nervous system, or death. Although all persons using these products on their hands are at risk, young children who accidently ingest these products and adolescents and adults who drink these products as an alcohol (ethanol) substitute are most at risk for methanol poisoning.
