1. Multiple employees have tested positive in the facility. The employer is not allowing exposed employees to quarantine. 2. Employees are allowed to return to work after testing positive. The employer is not requiring employees to provide a negative test before returning to work. 3. The delivery

3. Truck drivers are not provided cleaning supplies to sanitize the trucks before each shift.
4. Located at the facility, emergency evacuation muster point is in front of natural gas pipes.
