3. High touch surfaces in the facility such as door knobs, scanners, toilets, and faucets are not being cleaned on a routine basis.
4. Three of the electric lifts in the facility have damage to the cord's outer jacket exposing internal wiring.
5. A surge protector by the desk near the scanners has a damaged cord that was repaired using scotch tape.
