a speed limit. Forklifts are missing seat belts. Employees driving forklifts forward with load of pallets blocking their view. A metal ramp is used to access dock area. Ramp is very unsteady and also when wet very slippery. Forklifts often slide on the ramp. There are no dock lights in place for trucks driving into dock at night. Trailers are parked on a dirt road that is not secure causing them to sink into the ground and become unstable. There is poor housekeeping in the grinding and pallet fabrication area. Wood dust covers surface and not being cleaned daily . Bathrooms are very unsanitary and are not routinely cleaned. Bathrooms do not always have toilet paper and paper towels. Employees have tested positive for COVID-19 and employer has not informed employees that are close contacts about quarantine, testing, and return to work protocols. Employer has not provided masks and is not requiring use of masks in the work place to help minimize spread of COVID-19 Employer has not set up work stations six feet apart and has not trained workers to maintain at least a six feet distance from co-workers to help minimize spread of COVID-19. Hand sanitizer is not available in the work stations, to help minimize spread of COVID-19
