1. Employees are required to use electrical outlets and other equipment that have energized exposed electrical wiring. 2. Employees are required to work with dock plates that are not properly maintained and are creating safety hazards. 3. Employees are allowed to operate lift trucks without proper training.

4. Management allows employees to operated lift trucks in an unsafe manner. 5. In areas were lift trucks are used there is a lack of safe pedestrian walkways. 6. The lack of proper housekeeping creates many slip, trip, and fall hazards. 7. Employees were required to work with and around pallets that are not properly stored. 8. The fire extinguishers are not inspected regularly. 9. The employer does not have a Covid-19 prevention program that addresses sanitation. 10. Dock plates have not been repaired 11. Untrained operators are allowed to drive lift trucks 12. There have been near-miss and forklift accidents 13. Drivers do not slow down at blind spots 14. No signage 15. No pedestrian walkway 16. Broken dock desk, employees have been injured
