A bartender at Riverside Bar and Grill on Olde Oneida Street in Appleton has tested positive for COVID-19. The employee tested positive for the coronavirus on Sat May 23. 1. On 7/13/2021, an employee tested positive for COVID-19. The employee returned to work on 7/19/2021 still symptomatic with headache and fever. When the employee asked to go home because they were feeling ill, management denied the request. Return-to-work criteria was not met before the employee reentered... the workplace, exposing employees to SARS-CoV-2, the virus that causes COVID-19. 2. Employees do not have appropriate PPE while dealing with suspected COVID-19 positive residents, including but not limited to, respirators. 3. Employees are not provided FDA approved surgical masks for daily use.

Caregivers are restricted to consuming food and beverages in their break rooms. Other employees are not restricted where they are allowed to consume food and beverages. Employees allowed to consume food and beverages throughout the facility are not maintaining, at least, six feet distance or separated by... a physical barrier from other employees and residents, potentially exposing employees to COVID-19.

The employer is not implementing adequate safety practices to protect its employees from the spread of SARS-CoV-2 such as, but not limited to: 1. Screening employee and visitor temperature and assessing symptoms prior to entering the workplace 2. Providing facemasks and enforcing their use for all employees... and visitors where appropriate 3. Increasing air exchange in the buildings 4. Increasing the frequency of cleaning commonly touched surfaces 5. Enforcing social distancing requirements. 5. Informing employees when co-workers have tested positive for SARS-CoV-2 6. Performed deep cleaning in facility areas where SARS-CoV-2 positive employees recently worked or visited.

1. There are live wires exposed coming from or near the 160 Grinder, exposing employees to shock hazards. 2. There are live wires exposed coming from the junction box by the bucket elevator in the pit, exposing employees to shock hazards. 3. The walking paths are not... clearly painted, exposing employees to struck-by hazards from forklifts and falling product if they are outside of the walking path. 4. When super absorbent materials are grinded, the fibers go into the air, exposing employees to the health hazards of inhaling the fibers. 5. Employees are not trained on how to operate forklifts. 6. Forklifts are not inspected prior to being put into service at the start of the workday. 7. Employees must insert their entire body by crawling into the bag house vacuum and shaking the dust off the bag on a daily basis without confined space training, an attendant, or atmosphere monitoring. 8. Forklifts are operated inside the building and are accumulating exhaust vapors due to lack of ventilation. Employees are experiencing headaches as a result.

The employer is not following CDC guidelines and ensuring any employee or office personnel are isolating once having tested positive for the COVID-19 virus and/or coming into close contact with someone who has tested positive for the COVID-19 virus. Recently the Office Manager, PII, continued to work... at the facility during non-work hours after having tested positive with the COVID-19 virus. The majority of the office staff and multiple shop employees have since tested positive for the virus. And one employee was hospitalized with the virus.

On March 12, 2021, restaurant management was aware of an employee that was symptomatic and scheduled to get a COVID-19 test, and allowed that person to work on... March 13, 2021 while symptomatic, potentially exposing their co-workers to SARS-CoV-2 virus. 2. Both employees and management routinely do not wear their mask correctly, and pull it down to communicate, potentially exposing their co-workers to SARS-CoV-2 virus.
