1. The use of masks is not being enforced at the facility. Employees have been informed that the use of masks is mandatory but mangers do not enforce the use of it and it is up the employees to wear it or not.

The employer does not maintain an Infectious Disease Prevention and Preparedness Program. Social distancing is not maintained. The majority of employees are not wearing masks. Sanitation is deficient.
