1. The employer does not have a COVID-19-specific pandemic exposure control plan in place at the facility and the current pandemic plan is not being followed thus exposing employees to possible infection from SARS-CoV-2. 2. When staff are not interacting with patients, use of face masks/facial coverings is being not being enforced throughout the hospital and in ambulances thus exposing employees to respiratory hazards, including possible infection from SARS-CoV-2. 3. All employees who are required to wear respirators have not been initially fit-tested prior to being required to wear respirators thus exposing employees to respiratory hazards, including possible infection from SARS-CoV-2. 4. For employees who are required to wear respirators, the employer has not selected and made available respirators from a sufficient number of respirator models/types (e.g. half and full-face elastomeric respirators, powered-air-purifying respirators, supplied air respirators, etc.) so that the respirator is acceptable to the user, thus exposing employees to respiratory hazards.
