A strong chemical odor is present at the entryway to the facility, employees should be wearing respiratory protection, but they are not. Employees are exposed to flying sparks and dangling metal parts but are not being protected. There was an employee death from COVID-19.

1.) The company is not ensuring social distancing, and correct face covering/ mask use at the workplace. 2.) A worker reported a positive COVID test, however the company did not inform workers who had worked closely to the infected worker to quarantine for 14 days. 3.) The
facility was not cleaned sufficiently and according to CDC guidance before or after the employee became ill. 4.) The employee rest room is dirty and is not cleaned sufficiently. 5.)The facility does not provide hand sanitizer and adequate facilities for hand washing. 6.) Ventilation in the establishment is not sufficient to protect workers from transmission of COVID-19.
