1. The employer is not testing residents for COVID upon arrival to the facility or placing the residents in quarantine resulting in an outbreak of COVID among several residents. 2. Dietary employees are delivering food to COVID rooms and not given N95s to wear. 3. Employees are … See More
only being offered one N95 that does not have any NIOSH certification markings. 4. Employees are being fit tested with facial hair, passing the test and being allowed to wear their N95s. 5. Employees who cannot pass a fit test on the only respirator in the facility are required to being in their own respirators to use while at the facility. 6. The employer has not provided copies of the SDS for the chemicals that the housekeeping employees are using to disinfect the facility for COVID. Employees are unable to confirm if the chemicals in use are registered on the EPA list as effective virucides for COIVD. 7. Employees are not being told when a resident is awaiting a test result (PUI) and are caring for the resident with only a surgical mask. 8. Employees have requested a copy of the 300 logs and copies have not been provided. 9. Several employees have traveled out of the country and returned to work without quarantining.

stop any further activity with this entity. I found a phone number for the company Subgenix Products in the bottle. 800-609-4174 and called it. The receptionist answered in a foreign language which sounded oriental and I also heard what sounded like Spanish. When I requested an English speaking person to assist, they hung up the phone. I called again with the same results.
