1. A clinical staff employee developed symptoms of headache, loss of taste and smell after working closely with a doctor that had recently tested positive for covid-19. 2. The employee reported the situation to the office manager and requested a covid-19 test. The office manager replied, No,
it's just allergies. The employee continued seeing patients and discussed the situation with coworkers unmasked during lunch.

1. An employee in the housekeeping department (laundry) has refused to wear a mask during the entire pandemic and no one will address the situation. Other employees have been reminded to wear their mask and to wear it properly. Employees are exposed to her during these times.

1. The covid-19 testing door/area is the same door employees use to enter and exit the building. 2. An employee was tested for covid-19 due to possible exposure and symptoms and was instructed to return to work while awaiting the test results instead of isolating or quarantining
until the results were known. 3. Employee went home to isolate because patients with symptoms are not permitted to enter the clinic and expose other people.

1. Owner tested positive for COVID. Employees have been told to keep that a secret. All staff were exposed, including management. No one has been put on quarantine. 2. Business was not closed down for cleaning. Staff continue to interact with hundreds of customers.

1. Fifteen employees, 13 counselors and 2 trainers, were in an upstairs room roughly 12 X 12. No masks and very close contact to others' faces. Pizza was shared among everyone on Tuesday and Thursday and there was no recommendation from either trainer to go wash or
sanitize hands. A majority of the staff did not leave the room to wash their hands and did not use any form of sanitizer before using their hands to pick up the pizza. There was no recommendation of social distancing from either trainer and upwards of 8 counselors were observed to be sitting side by side on the floor during a training video, not wearing masks on 7/2/20.

1. While we are required to wear a mask on job sites if the client requests it, there have been no attempts at enforcing social distancing/mask wearing/sanitizing surfaces from the employer or coworkers. 2. There is a sign on the door for doing a health screening at
the beginning of the day, but it does not seem to be strongly enforced.
