1. Emergency Center staff required to clean COVID positive room with only sanitary wipes. 2. The floors are never cleaned between patient's and trash is not emptied. 3. Staff does not have the appropriate cleaning supplies to protect against COVID.

a remote work policy for individuals who do not directly interact with patients but staff is told that it is disruptive to the doctors and wont be done even though the work can be done from home. 3. Not properly disinfecting and cleaning between each patient due to the clinics heavy schedule to see patients. 4. The clinic is a hectic traffic jam, overloading the clinic waiting areas with patients and support staff which doubles the amount of foot traffic in the clinic and not ensuring proper social distancing. 5. There is not a COVID-19 Preparedness Action Plan posted anywhere, or in writing for review by any of the staff. 6. Employee have not been properly trained regarding disinfection of their workspaces. 7. PPE is not readily available to every employee, as we are "highly encourage" to supply our own. 8. It is not posted anywhere, as to where an employee could file a complaint if they feel it is unsafe.

machines are not being cleaned daily. 3) No gloves or extra PPE were provided to the staff who had to clean and organize a common areas in preparation for new carpeting being installed and most of the staff had to work in close contact (less than 6ft) with each other while performing this task, not allowing for proper social distancing. 4) Not receiving notifications of positive cases, not informing the staff of being exposed to employees who tested positive for COVID-19, most employees find out from other employees.
