same day. 5. Doctor does not wear mask when in front office, most office staff removes face coverings multiple times during shifts to speak. 6. Staff is not actively disinfecting waiting room, shared clip boards, pens, thermometer, bathrooms, office supplies, overall work stations daily or weekly. (Work sheets falsely recorded to show being completed, but only a room spray is being used) 7. Basement is a hazard and not properly lighted
