1. Surgical procedures are performed on patients that are COVID-19 positive, in operating rooms that are not negative pressure - exposing staff, physicians, and other patients. 2. There was an organ procurement on a COVID-19 positive patient. The donor patient had not fully recovered (was still positive)
and was deemed "highly infectious," putting everyone involved in harm's way.

1. The filters on negative pressure units in COVID rooms are not being changed, they are full of dust and debris.

1. Very common to see staff not wearing masks, nurses, but also physicians. common siting in the ER, and on inpatient units throughout the hospital. 2. Physicians are not being held to account for Covid-19 PPE polices and are exposing McLaren Staff. 3. Patient testing is done
using the rapid test. Which on many occasions has found to be inaccurate, and the patients are found to be positive with Covid-19 after all. After they have been admitted to semi-private rooms with patients without Covid-19. Or admitted without isolation orders, exposing countless employees and patients for, sometimes, days. 4. An employee who has worked at the hospital for 30 years recently died at the hospital as a Covid-19 patient. 5. McLaren Flint has never done screening at the door they don't take our temperature they simply say any symptoms today and people walk by them and they say have a good day. 6. I look around me behind the desk at the nurse's station and I have coworkers pulling their mask down constantly and still only wearing surgical masks. This includes physicians and nurses not wearing masks properly. 7. McLaren continues to do elective surgeries and when I ask patients did you get a Covid test before your surgery half of them say no. 8. There are times when we suspected a patient had Covid after they were admitted and the Chief Medical Officer tells the physicians to not test because he doesn't want anyone to know they got it while they were there or that they're being irresponsible.

1. Hospital is having maintenance staff move patient lifts from overhead in ICU Covid positive rooms because there are not enough lifts available. It is a danger to staff and patients. 2. Staff must risk infection and climb a ladder to move lift without 3 points of
contact on the ladder and repeat in the next room.

1. Employee was sent to work 9 south (step-down, covid designated floor) on 9/22 and to ICU (also a covid floor) on 9/26 without n95 mask. Employee was required to go there and work without proper PPE. Positive incubated patients are transported through the hallway by employee
and all they had for protection is a simple mask!!!

1. No proper fitting N95 masks available when exposed to possible Covid 19 patients. We are fit tested with the appropriate mask, then on the next shift (I was told we get a new mask daily!) they give us a different fitting mask that do not properly
fit. I contracted Covid 19 due to this. Managers were notified and we were told "we don't have enough supplies" - from an ED nurse.

1. 9T continues to have negative pressure rooms that are pumping Covid air into the halls and other floors. No action has been taken and continue to risk lives of staff. We have had 2 deaths and many people sick. Message from LARA was it's a problem
and we need to push the issue. 2. Also no N95 masks or N95 fitting for 7c. This floor had a Covid break out on 9/18 with 9pts and 5 RNs so far that have been tested positive. The employer started to fit test on Saturday 9/19 and test patients. Many emails demanding they test the RNs also and they agreed. This should have never happened and all employees should have been fit tested and provided N95 at this point.
