1. It is alleged that workers have tested positive for COVID-19, the CDC Guidelines, such as but not limited to, the Interim Guidance for Businesses and Employers to Plan and Respond to Coronavirus Disease 2019 (COVID-19) are not being followed. 2. The employer does not provide nor
do they require the wearing of mask by staff due to COVID-19. 3. The employer has not performed a hazard assessment for personal protective equipment (PPE) such as, but not limited to, safety shoes, safety glasses and/or goggles, and hard hats. 4. The first aid is not stocked. 5. Battery acid leaks on the floor from batteries that are stored throughout the facility. Warning signs are not posted and the spills are not cleaned up appropriately. 6. Employees have to manually lift (and retrieve) materials that can weigh 40 lbs or more from a ladder onto a the top levels of the racking system. Employees working below are not required to wear hard hats when overhead hazards are present. It is alleged a worker was hit in the head by falling product or similar and nothing was done. 7. An eye wash and/or quick drenching device are not readily available when employees get particles or similar in the eyes and where battery acid is present. 8. The employer doesn't have a site specific written hazard communication program in place and training isn't performed with all staff. 9. Employees are exposed to slips, trips, and falls due to the toilets in the restrooms which overflow and are in disrepair. Warehouse/factory staff are only allowed to us two (2) of the three (3) available restrooms. 10. Restrooms are not clean due to waste that overflows from the toilets. 11. Restrooms do not have soap and paper towels available. 12. There are no blind spot mirrors in the facility for the fork truck operators. It is alleged that at least one near miss (an employee was almost hit by a fork truck) has occurred and the employer didn't follow up on the near miss.
