Unsanitary conditions - Bedding, rooms, etc not cleaned. Sheets and bedding dirty/moldy. No COVID plan or cleaning requirements in place Food carts have mold growing on them, unsanitary conditions in kitchen Electrical outlets are burned/faulty and still in use
