Hazard Description: Employee concerns over recent COVID-19 outbreak at workplace. Employee alleges several employees contracted COVID-19 at work after an employee was permitted to come to work the day after they had a fever. The employee in question returned to work on April 12, 2021 after allegedly... See More having symptoms of COVID-19 to include a fever on April 11, 2021. Employee alleges the employer conducted a shift meeting on April 12, 2021 and employees wore masks but were not advised or required to socially distance six feet or more while attending this meeting in the same room. Employee concerns raised about cleaning procedures prior to outbreak. Employee reports that they do not believe the employer has health officers who are responsible for ensuring COVID-19 protocols are met. Employee has concerns as to whether or not proper contact tracing was conducted by the employer. Employee is unsure if employer documented illness on 300 logs. Location: Throughout establishment.

Employees operate powered industrial trucks unsafely, including not stopping and looking at intersections, not obeying safe distances between powered industrial trucks, and not sounding horns for... See More warnings. 3. An employee complained of back pain, and the employer wanted to send this employee for a drug test, which discourages injury reporting. 4. Pallets are crooked on the racks and could get struck by other pallets getting placed onto or removed from the racks, especially on the high rack levels. This could result in the pallet or boxes falling and striking employees below. 5. The LED lights are very bright making it hard for powered industrial truck operators to see when placing materials or retrieving materials from fifth level storage areas. 6. There is no social distancing for infection control in the cafeteria, locker room, and when entering to start work. At the start of the work days, about 80 people come in at one time and congregate close together.
