1. Poor mask compliance (customers): estimate at 50 percent. There is no mask checker at the entrance. 2. Allowing employees with symptoms to continue working. There was a cashier that was coughing and was allowed to continue working for two days. Then she was gone and tested positive. 3. No symptom check of employees before each shift: no temp check and no list of symptoms. This has been the norm even with the worsening pandemic. 4. One fatality ( patient was a manager and was recovering at home from COVID). The remaining management is trying to maintain the death was possibly non COVID related and had requested an autopsy. This supposed victim of COVID was just found this AM dead at his home while recovering from COVID. 5. Several cases of ill employees who are all of a sudden gone for two weeks and there is no information given to other co-workers as to if they should take extra COVID precautions. 6. When two of the managers first tested positive, there was no obvious written notification for employees or customers to let them know of the increased level of risk. Then I noticed a small memo posted on the wall of the Subway Shop located within the store. It stated that the two brothers tested positive for COVID (1 has since died). This was not put in a central place for all employees to be able to see it, like where you punch in or in the break room.Source: Osha.gov | Receipt Date: 2020-12-10
