The facility is not adhering to CDC guidelines for a safe and clean environment for the elderly who have been affected the most by the COVID-19 pandemic. Staff and visitors are not wearing masks. There are no hospital grade sanitary wipes to clean and disinfect patient care areas. The facility is infested with roaches in the nurses station, rooms, kitchen, and hallways. There is mold-like material in both nourishment rooms and visible in bedrooms 207, 216, 118, 123, 219, 206, and 124.
