(1) The employer is not conducting proper screening for newly admitted patients into the B2 North facility. Patients with positive COVID-19 are admitted into the department that is not equipped with isolation rooms to handle COVID-19 cases, and are allowed to interact outside their rooms exposing staff and other residents to the hazard of contracting COVID-19. (2) The employer did not conduct proper staff and residents screening when confirmed COVID-19 cases or contact with suspected COVID-19 cases were reported to the employer. (3) The employer did not take proper infection control measures including cleaning and disinfection when patients and staff members with confirmed or suspected COVID-19 are known to have been in the room. In one incident, a patient was placed in a room of a COVID-19 positive patients leading to the new room occupant and an attending staff contracting COVID-19 right after the transfer of the patient to a COVID-19 isolation unit. (4) The employer did not effectively communicate with staff members who were exposed to patients with COVID-19 resulting enforce the quarantine policy for employees
