The Virginia Positivity Rate for COVID-19 transmission has been above 5 percent; therefore, all employees, regardless of vaccination status, are required to wear face coverings when indoors. Item No.2: The employer does not ensure unvaccinated employees maintain six feet of distance between one another. Item No.3: The employer allowed a confirmed or suspected COVID-19 employee to report to and/or remain at the work site. Item No.4: The employer did not notify employees of a confirmed or suspected COVID-19 co-worker within 24 hours of themselves being notified. Item No.5: The employer did not ensure all common spaces, such as frequently touched surfaces and doors were cleaned at least once during or at the end of the work shift.
