1) Electric motorized pallet jacks are in disrepair throughout the facility. Employees are required to operate this equipment with damaged wheels, causing a struck-by hazard for the employee. 2) The positive and negative battery cables on the electric motorized pallets are damaged exposing live wires. Some of the electric motorized pallets are missing the positive and negative covers for the conductors, exposing employees to a shock hazard. 3) The cherry picker has a bent outrigger and employees are required to still operate it. Employees are exposed to a struck-by hazard from operating this piece of equipment. 4) In freezer 2, icicles are hanging from the ceiling that pose a struck-by hazard for employees. 5) Storage racking throughout the facility has severely bent supports and missing bolts, exposing employees to a struck-by hazard. 6) The employer has not implemented a COVID-19 prevention program including good housekeeping practices of regularly cleaning and disinfecting surfaces, equipment, and other elements of the work environment.Alleged Hazards: 6, Employees Exposed: 50
