1. The employer is not notifying employees of exposure when close contact to a Covid-19 positive case occurs, including exposure(s) to other employees, residents/patients, etc. 2. Physical distancing practices are not required and/or enforced as employees are asked to attend face-to-face meetings and physical barriers have been removed. 3. The employer does not provide and/or require the wearing of facemasks that are FDA-cleared, only cloth face coverings are provided to employees. 4. The employer´s COVID-19 plan is not monitored and/or updated as needed to ensure ongoing effectiveness. 5. The employer does not screen each employee, patient, resident, visitor and other non-employees entering the facility to determine whether a person is COVID-19 positive or has symptoms of COVID-19.Alleged Hazards: 5, Employees Exposed: 120
