1. Failure to develop a COVID-19 preparedness and response plan that can be readily provided to employees. 2. I started there and worked for 2 days, there was no COVID preparedness, signage or any effort to mitigate contact. I was being trained sitting inches from my trainer, sharing a phone, keyboard and workspace. 3. No PPE, not asking any contractors who came in to wear masks, when a contractor came in he was told that he did not have to wear a mask. 4.
