1) Forklift Operators have not received training or been evaluated for operational skill. 2) Forklift Operators are not sounding horns are blind intersections. 3) Forklift Operators are not wearing seatbelts. 4) Forklift Operators are travelling forwards with obstructed views. 5) Employees are not allowed to help injured or sick employees. In July 2020, 2 employees required medical treatment and employees were not allowed to leave the Production Line to help them. 6) The floor is slippery throughout the Production Area. The HVAC system is not removing moisture from the air and the moisture is condensing on the floor surface. 7) Employees working with spoiled meat were not provided additional protective equipment. 8) The guards on the Box Closing Machine at the end of the Production Line have been removed. 9) Employees must sign out and sign in to use the restrooms. Restrooms are not readily accessible for employees. 10) The employer has not developed and implemented OSHA and CDC recommendations for COVID-19 in the facility, even though an employee has tested positive. The sanitation station is not adequate for the number of employees during shift changes and breaks. The location of the sanition station prevents social distancing.
