1. Employee bathrooms are not maintained in a clean condition. 2. The stairs which are used to access the 2nd and 3rd floors are not maintained in a safe condition. 3. The employer has not implemented a COVID-19 prevention program including things such as wearing face coverings and or not wearing face coverings properly, training, social distancing, etc. 4. The employer has not implemented a COVID-19 prevention program including good housekeeping practices of regularly cleaning and disinfecting surfaces, equipment, and other elements of the work environment. 5. The employer's COVID-19 prevention program is not adequate in that it does not have adequate screening to ensure potentially exposed employees are separated and sent home.Alleged Hazards: 5, Employees Exposed: 10
