1. The floor drains backing up regularly. The past week septic water has been coming up out of the floor drains and the issued is neglected. 2. There are multiple trip hazards around the entire building in regularly trafficked work areas. 3. Lockout/tagout is not practiced. 4. Allowing sick employees to work and not following CDC guidelines. 5. There are no OSHA or employee rights posters anywhere on the premises. 6.
