3205: Employer is not requiring face covering since 6/15/21 and they are optional. Non-vaccinated employees are not wearing face covering. Employee who have had Covid are not getting vaccinated. There are no facility services to do daily cleaning of common areas such as restrooms, lobby and copy room machines, door handles, etc. No signs on doors telling customers to put on a mask.Alleged Hazards: 5, Employees Exposed: 1
