1. Employees are not provided respiratory protection while working in a cloud of silica dust on the second and third floors. The dust is created by dry cutting bricks. 2. There's no guardrails on scaffolds on the second and third floors. Employees are working on the scaffold next to the exterior wall openings. 3. Guards are missing on two portable grinders used for cutting metal on the second and third floors. 4. Employees are not properly using step ladders. The are leaning the ladders up against the walls. Also, they are using them next to the opening of the exterior wall. 5. Strain relief on extension cords in the gang box located at the entrance to the building. 6. A COVID-19 pandemic plan has not been developed and implemented for employees to include employee wellness checks, quarantining employees, informing employees for possible exposure to the virus, employee returning to work, social distancing, personal protective equipment, disinfecting/sanitizing, and employee training.
