1. The employer has not effectively developed or implemented an Infectious Disease Preparedness and Response Plan for COVID-19. The employer is allowing people that were sent home for being infected with COVID-19 to come back to work while still showing symptoms of being infected with the virus. 2. Social distancing is not enforced or maintained at the location. The employer is allowing gatherings in enclosed spaces of more than three people after company policy stated that there should not be such gatherings. 3. The use of masks or face covering is not enforced. Employees working in close proximity to others, for example, inside offices are not wearing masks. 4.
