1. Forklift training has not been conducted with employees operating forklifts. 2. The face plate guard has been by-passed on the wheel dressing machine located in the new addition area. 3. A homemade key is used to by-pass the gate to access the robotic section of the Walter Vision and Walter Mini machine in the Old Shop Area. 4. Eye protection is not enforced within the production area. 5. A COVID-19 pandemic plan has not been developed and implemented for employees to include employee wellness checks, quarantining employees, informing employees for possible exposure to the virus, employee returning to work policy, social distancing, personal protective equipment, disinfecting/sanitizing, and employee training.
