The employer has not effectively developed or implemented an Infectious Disease Preparedness and Response Plan for COVID-19. The use of masks or face covering is not enforced. Contact tracing has not been conducted with employees that has been in close contact with positive COVID-19 cases, so they could either quarantine or get tested. The employer has not provided adequate sanitization at the facility such as, common areas and work areas, after confirmed COVID-19 positive cases.
